Minor New Card (In-Person)

Clinic exam with both required physician approvals for minors. Parent or guardian must attend the appointment.

Adult New Card (In-Person)

In-clinic evaluation with full OMMA filing for new patients. Requires valid ID, email access, and card arrives in 2–3 weeks. Valid for 2 years.

Virtual New Adult Med Card

Adult Renewal (In-Person)

Clinic renewal visit with full application processing. Bring valid ID and proof if applying for OMMA discounts.

Renewal (In-Person, Cash)

Clinic renewal visit with fee paid in cash at appointment. OMMA fee is separate and submitted online.

Medical Marijuana

New Card (In-Person, Cash)

Clinic evaluation for first-time adult patients, fee paid in cash. OMMA fee is separate and paid online.
