Virtual Adult Renewal
Quick telehealth visit with full renewal filing. We handle your OMMA application once you provide login details and photos.
Virtual New Adult Med Card
Telehealth evaluation for a first-time card. Includes full filing with OMMA once ID, residency proof, and details are provided.
Virtual Renewal – You File
Telehealth renewal with physician recommendation provided. You handle OMMA filing and payment directly online.
Minor New Card (In-Person)
Clinic exam with both required physician approvals for minors. Parent or guardian must attend the appointment.
Adult New Card (In-Person)
In-clinic evaluation with full OMMA filing for new patients. Requires valid ID, email access, and card arrives in 2–3 weeks. Valid for 2 years.
Adult Renewal (In-Person)
Clinic renewal visit with full application processing. Bring valid ID and proof if applying for OMMA discounts.

OMMA fees are additional and paid when we submit your application. Current OMMA rates referenced in your notes: $104.30 standard; $22.50 with Medicare, Medicaid/SoonerCare, or 100% disabled veteran status (bring proof). Rates are set by the state and may change.
